Build intake forms, tax organizers and year-end checklists, or start from one of 12 ready-made templates. Clients fill them in from an email link, the client portal or your own website. Answers land on the client, the filing and the step, and file uploads go straight into their documents.

Nothing is written to the client until someone on your team approves it.
How It Works
Start from a ready-made template or a blank form. Add sections, questions, file uploads and rules in the designer, preview it as the client will see it, then publish.
Email a private link to one client or up to 2,000 at once, show it in the client portal, or put it on your own website. Tie it to a client, a filing, a filing step or a walk-in.
Answers arrive in the Forms inbox. See exactly what would change on the client, apply it with one click, or send the form back for a correction.

What Forms Can Do
Show or hide questions based on earlier answers, with AND / OR rules. Rules can also use the client type, filing type, tax year, the client's services and your custom fields.
Let the client add a set of questions again and again, for each dependant, rental property or employee, without you guessing how many to allow.
Files a client uploads in a form are checked and saved straight into that client's documents, so slips and statements never sit in an email.
Link a question to a client field such as address, phone, email, date of birth or your own custom fields. Staff see a before-and-after and approve every change.
Send one form to up to 2,000 clients filtered by type, service or group. Each person gets their own private link, and last year's answers can be carried forward.
Signed-in clients fill forms inside the client portal. Public forms can go on your own website, allowed only on the sites you list, with your firm's branding.
Automatic reminders chase anyone who has not finished. If an answer is wrong, send the form back with a note and the client corrects it.
Attach a form to a client, a filing, a filing step or a walk-in. Send it from the filing step itself, and the answers sit right next to the work.
Email verification codes, forced on any form with a sensitive field. Sensitive answers are masked for roles that should not see them. Every response keeps an audit record, and public forms have bot protection.
FAQ
A form designer built into MyCPACRM. You build intake forms, tax organizers and checklists, send them to clients by email, the client portal or your website, and the answers and uploads come back into the client's record instead of your inbox.
No. There are 12 ready-made templates, including personal and business onboarding, a T1 organizer, a US 1040 organizer, corporate T2 year end, GST/HST period, payroll new employee, T3 trust, T5013 partnership, year-end documents and walk-in intake. Using one copies it into your library to edit as your own.
Never on its own. When a question is linked to a client field, staff see the value on file next to the answer from the form and choose to apply it. Blank answers are never treated as a change, and a submission never creates a client by itself.
Yes. Clients who use the client portal fill forms there while signed in. Public forms can be embedded on your firm's website, and only on the websites you list for that form. Public submissions arrive in the Forms inbox, flagged when the email matches an existing client, for your team to link.
Any question can be marked sensitive, such as a SIN. A form with a sensitive field always requires a verification code sent to the recipient's email. Sensitive answers are masked for staff roles without permission to see them, never included in emails, and every response keeps an audit record.
Yes. A form can be sent to the same client again, for example your yearly organizer, and last year's answers can be carried forward so clients only update what changed.
No. Custom Forms is included in every MyCPACRM plan, with no per-form or per-response fees.
